INTAKE AND ADMISSIONS
We recognize that the decision to seek treatment is not easy and that reaching out for help is often difficult. At BALANCE we are here to assist you. We make every effort to guide you through the process as easily and smoothly as possible.
Our intake process begins simply with a phone call to our main office or an email inquiry to our website. We will gather information about your concerns and answer any questions you may have. During the initial phone call you will be given a list of the required lab work we will need. You will be offered an in person appointment at your convenience. Should you wish to see our facility before making a decision about treatment we encourage you and your family to tour our center and meet with members of our team.
Our intake is a two-fold process and is typically two hours in duration. All clients meet with both a licensed psychotherapist and a registered dietitian. You will also be asked to sign a consent form giving us permission to contact any treatment providers or referents involved in your care. As part of the admissions process you will be asked to bring in your lab results from your physician as we require a full medical evaluation to determine the appropriate level of care to best meet your needs.
If you are contacting us from out-of-state or from a residential setting we will arrange to conduct all necessary admission interviews by phone.
After the intake is complete, we will reach out to your current outpatient team to discuss our assessment and coordinate care in accordance with our collaborative treatment recommendation. After consulting with your treatment team we will establish a personalized treatment plan best suited to you.
